COMMITMENT TO PAY

By completing and returning this form you are signing up your child to attend the summer workshop selected above. You will be expected to to pay the full fee within 14 days of receiving payment details. These will be sent to you by email on submitting your application. Your place will be reserved for 14 days and may be passed on to another performer if payment isn't made in this time.

CHILD PERFORMERS LICENCE

For students attending Shrek Musical workshop or Grease, we are required by law to provide the local authority with details of all our performers names, ages, addresses and schools attended in order for us to gain the appropriate license for our performance. This is to confirm that the performance has not affected attendance at school and that your child is not being paid to perform. The local authority (Rutland and/or Lincolnshire) Will hold this information for compliance and regulations for a minimum of 6 months.

MEDIA CONSENT
We would like to photograph our workshop to show students progress and share success. The final performance will also be photographed and filmed. Names of students or any other personal information will NOT be linked to our images.
