By completing and returning this form you are signing up your child up to attend a workshop hosted by RYDA. You will be expected to pay the full fee within 14 days of receiving payment details. These will be sent to you by email on submitting your application. Your place will be reserved for 14 days and is not guaranteed if payment isn't made in this time.

MEDIA CONSENT
We would like to photograph our workshop to show students progress and share success. The workshop will also be photographed and filmed. Names of students or any other personal information will NOT be linked to our images or footage.
